Racism and myths shape African-American women's views on depression care
If you're a black woman and you find you can't get out of bed because you're depressed, most likely you'll receive little sympathy. More than likely, you're seen as a "strong black woman" and people will be confused if you can't keep it all together. A study of African American women living in Portland, Oregon, reveals that racism and the expectation that black women be "the strong black woman," are obstacles in black women seeking care for depression. The story is all too familiar for Dr. Annelle Primm, who, as a psychiatrist, treats depression among minorities, particularly African American women. She and her family started a Baltimore-based company, Health Disparities Solutions (HDS) LLC, in 2003 to educate health professionals on the multiple ways in which health disparities impact not just minorities but the well-being of the entire nation. HDS consists of Annelle's father, Beny Primm, an anesthesiologist who has served as an adviser to three U.S. presidents, her sister Jeanine Primm, who has developed culturally relevant public health programs, and Annelle who in 1985 established COSTAR. The program was designed to provide specialized treatment for people with severe and persistent mental illness. Many of these patients were not receiving conventional outpatient treatment because they wouldn't show up for appointments or take their medication. The COSTAR program consists of a team of psychiatrists, social workers and nurses who provide home outreach to minority patients with severe mental illnesses such as schizophrenia. In addition to medical treatment, the team also makes sure patients take their medication and helps them with housing needs. The program was the first to implement such techniques in an urban setting. Though the program still exists, Annelle stepped down as director in 1995. Primm also created two videotapes: "Black and Blue: Depression in the African American Community" and "Gray and Blue: Depression in Older People" to address the stigma in the Black community associated with seeking mental help. "I used the medium of video to level the playing field in terms of literacy." says Annelle, who is also director of minority and national affairs for the American Psychiatric Association in Alexandria, Virginia. "In terms of educating people about mental health, we used people who had actually suffered from depression, so that it would come directly from them." Researchers with the study in Oregon found one issue dominated discussions about depression care - the participants' deep mistrust of what they perceived to be a "white" health care system. "These women were extremely wary of most depression treatments and providers they associated with 'white' systems of care. Although they acknowledged that violence, depression and substance abuse adversely affected their health, discussions about health care revolved around their perceptions of racism," said Christina Nicolaidis, M.D., M.P.H., principal investigator and an associate professor of medicine (general internal medicine and geriatrics), and public health and preventive medicine in the OHSU School of Medicine. "Based on our findings, we recommend health and mental health providers endeavor to better understand and acknowledge how racism informs the experiences and perceptions of their patients." The expectation of being a "strong Black woman" also was a significant barrier to recognizing depression and seeking care. Co-investigator S. Renee Mitchell has used this finding to launch a campaign asking: "Strong Black woman - what are you burying, your feelings or the myth?" The research team also has organized several community depression and violence awareness events titled "Redefining the Blues." Study participants expressed a desire for community-based depression programs that addressed violence and drug use and are staffed by African-Americans with "real-life experiences." In response to this request, the research team used their study data to create a community-based, culturally tailored depression-care program, which they currently are pilot-testing at Bradley-Angle House's Healing Roots Center, a drop-in center for survivors of domestic and sexual violence. Do you think the myth of the "strong black women" keeps women from seeking mental health care, or is it the potential for having a mental health care provider misjudge a black woman because of racist views? Or a combination of both? http://xml.channel.aol.com/xmlpublisher/fetch.v2.xml?option=expand_relative_urls&dataUrlNodes=uiConfig,feedConfig,localizationConfig,entry&id=868400&pid=868399&uts=1278511441 http://www.aolcdn.com/ke/media_gallery/v1/ke_media_gallery_wrapper.swf Your Hottest News Stories 01-July-10 - Crystal Gale Mangum, 31, the woman who accused members of the Duke lacrosse team of rape, held a press conference in her yard on Wednesday to announce that the new charges against her, involving arson and attempted murder, are false.
